Please review below some information about our payment and insurance policy.

Appointments & Cancellations
To make an appointment, please call our office administrator at (416) 927-9085. Special arrangements can be made for emergency treatment.
We require a 48 hour notice to cancel an appointment or a $50.00 charge will apply.
Payment
Payment is due as services are provided. At the conclusion of your appointment you will receive a statement outlining the services that were performed during your visit.
For your convenience, we accept Interac, VISA and Mastercard payments. We can also electronically submit your dental claim to your insurance provider or provide you with a generic dental insurance claim form that you can submit to your private insurer or employer.
When more extensive treatments are required, we often customize a payment plan depending on cost and length of treatment.
Dental Insurance
Patients with dental insurance are responsible for their percentage of the treatment costs as well as any treatments procedures that their insurance company does not cover.
At the conclusion of your visit you will be billed the entire treatment cost. We will assist you in filing and submitting your insurance forms and your insurance company will reimburse you directly.
You should be aware of the specific levels of coverage provided by your employer’s insurance such as:
- the annual maximum allowed per patient,
- the annual deductible (if any), and
- the percentage of coverage allowed for specific services (i.e. diagnostic, preventive, restorative, endodontic, crowns, bridges, specific types of fillings, etc.)
This information can be obtained from your employer’s employee benefits department.
